I went to a local counter top company that has a large inventory of Formica on hand in hundreds of colors. This place supplies most of the market around here for shops and home repair companies.
Look in the yellow pages for your town and see who sells Formica and they can (should) tell you who they are supplied by.
This picture shows the original Formica dash panel and the new one I cut. I like it because it won't cut and you don't have to staple it a thousand times. Glue it on with contact cement or a good 3M interior spray, trim with a router and a laminate bit and you're done.

AND .. 

1. There is an Adobe Illustrator file a graphic designer can use to alter and to print it (for example replace the C-535 and redo that with a CVX_17) . You need to have the Adobe Illustrator software.

2. The other one is a PNG file.
